3. The name of your home institution at time of participation

Students who studied with SFS from 1994 to the present and did not attend an SFS-affiliated institution (click here for a list of current affiliated institutions)*:
Please make your transcript request in writing and mail or fax directly to Boston University. Requests should include:
1. Your full name and Social Security Number2. Date of birth
3. Dates and numbers of the courses attended
4. Names and addresses of those receiving copies
5. $6 per transcript made payable to BU

Depending on where your request must be made, processing times will vary. Transcript requests made through SFS are generally processed within five business days. If your request will be made through Northeastern or Boston University, please consult the appropriate website for standard processing times in order to ensure adequate time for your request to be processed.
