It is important for safety and program quality that The School for Field Studies have comprehensive information regarding the physical and mental health of our students. To that end, we require that all our students complete an online Medical Review form and submit a Physical Exam form completed by their physician. Additional forms may be required depending on which SFS Center you are attending and/or your particular health history.

This process is designed to inform, not to exlude. Should we have concerns regarding a student's capacity to participate, we will contact the student and/or medical examiner to discuss them. Inaccurate or incomplete information on any of your health information forms may be grounds for rescinding an offer of admission, recalling you from the program or restricting your participation in certain activities. It may also affect the quality of your experience (as well as that of other participants) and/or your academic credit. Please inform us of any changes in your health status that occur after submission of health information forms.
