It is important for safety and program quality that The School for Field Studies has comprehensive information regarding the physical and mental health of its students.
To that end, we require that all students complete the SFS Online Medical Review and submit an SFS Physical Exam and Authorization Form. The SFS Physical Exam and Authorization Form must be completed by a physician, physician’s assistant, or registered nurse practitioner. Students who plan to dive as part of the Turks and Caicos semester or summer programs must complete a PADI Medical Statement and have it signed by a physician.
If a student has been in counseling for any reason within the past three years, an SFS Counseling Questionnaire must be completed by the student’s counselor. If a student is currently taking a psychotropic medication, an SFS Psychotropic Medication Questionnaire must be completed by the student’s prescribing medical provider. Additional medical paperwork may be required depending on a student’s particular health history.
The medical approval process is designed to inform, not to exclude. Should SFS have concerns regarding a student's capacity to participate, we will contact the student and/or healthcare provider to discuss them. If you have any questions or concerns about this process, please contact SFS headquarters and speak with a member of the Safety Department.
Inaccurate or incomplete information on any of your medical paperwork may be grounds for rescinding an offer of admission, recalling you from the program, or restricting your participation in certain activities. It may also affect the quality of your experience (as well as that of other participants) and/or your academic credit.
Please inform us of any changes in your health status that occur after submission of your medical paperwork.
The required SFS Online Medical Review and the SFS Physical Exam and Authorization Form as well as the SFS Psychotropic Medication Questionnaire (if applicable) and SFS Counseling Questionnaire (if applicable) are available on the right hand side of this page and also on the Admissions Section of the SFS website. Do not submit any medical paperwork to SFS until you are an accepted student.
