Medical Approval Process

It is important for safety and program quality that The School for Field Studies has comprehensive information regarding the physical and mental health of its students.

To that end, we require that all students complete the SFS Online Medical Review and submit a Physical Examination and Authorization Form. The Physical Examination and Authorization Form must be completed by a physician, physician’s assistant, or registered nurse practitioner who is not related to the student. Students who plan to dive as part of the Turks and Caicos semester or summer programs must also complete a PADI Medical Statement and have it signed by a physician.

If a student has been in counseling for any reason within the past three years, an SFS Counseling Questionnaire must be completed by the student’s counselor. If a student is currently taking a psychotropic medication, an SFS Psychotropic Medication Questionnaire must be completed by the student’s prescribing medical provider. Additional medical paperwork may be required depending on a student’s particular health history.

The medical approval process is designed to inform, not to exclude. Should SFS have concerns regarding a student's capacity to participate, we will contact the student and/or healthcare provider to discuss them. If you have any questions or concerns about this process, please contact the SFS Director of Safety and Student Life.

Inaccurate or incomplete information on any of your medical paperwork may be grounds for rescinding an offer of admission, recalling you from the program, or restricting your participation in certain activities. It may also affect the quality of your experience (as well as that of other participants) and/or your academic credit.

Please inform us of any changes in your health status that occur after submission of your medical paperwork.

The SFS Online Medical Review Form must be completed and submitted through your "My SFS" account. Click here to log in and complete this form. The additional required forms are available on the right hand side of this page and also on the Accepted Students page of the SFS website. These include: (1) Physical Examination and Authorization Form; (2) Psychotropic Medication Questionnaire (if applicable); (3) Counseling Questionnaire (if applicable); and (4) PADI Medical Statement (if applicable). Do not submit any medical paperwork to SFS until you are an accepted student.
